Peer Support
Peer Support Services are structured and scheduled activities for adults aged eighteen (18) and older with
a diagnosis of Mental Health and Substance Use disorders. Peer Support Services emphasizes personal safety, self-worth, confidence, and growth, connection to the community, boundary setting, planning, self-advocacy, personal fulfillment, and development of social support, and effective communication skills. Services emphasize the acquisition, development, and expansion of rehabilitative skills needed to move forward in recovery.
Phoenix Transportation (NEMT)
Coming soon! Non-Emergency Medical Transportation (NEMT) provides Medicaid beneficiaries with transportation to medical appointments. NC Medicaid Managed Care health plans provide NEMT services for their members.
Community Health Workers
Coming Soon! Community Health Workers serve as a liaison/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Personal Care
Coming Soon! In-home personal care services will assist seniors and other adults with activities of daily living and the basic skills needed to live independently at home.
Personal care services can include bathing, dressing, grooming, oral hygiene, help with mobility, eating assistance and toileting care.
Companion Care
Coming soon! Companion Care focuses on assisting clients with daily household tasks, errands, transportation and social companionship.
